  10.  AB    OBJECTIVE. To evaluate the impact of the 1993 CDC HIV classification
  11.        system versus the 1987 definition on the number of cases and on survival
  12.        of AIDS patients through the retrospective analysis of data from a
  13.        prospective cohort study of patients treated with zidovudine. A total of
  14.        3515 patients enrolled in the National Registry of zidovudine-treated
  15.        patients between July 1987 and December 1991 were analysed. RESULTS. We
  16.        compared numbers and survival probability estimates (using the
  17.        Kaplan-Meier method) for patients satisfying the 1993 CDC case
  18.        definition compared to patients fulfilling the 1987 CDC classification.
  19.        According to the new classification system the number of AIDS cases in
  20.        the study population would increase by 188%. While the median survival
  21.        of patients meeting the 1987 definition was 24 months, at the end of 57
  22.        months 47% of patients meeting the 1993 definition were still alive.
  23.        Among the patients meeting the laboratory criteria for AIDS diagnosis
  24.        using the new definition (CD4+ count < 200), the presence of an
  25.        AIDS-defining illness was a strong independent predictor of death.
  26.        CONCLUSIONS. The application of the new definition results in a
  27.        considerable increase in the number of cases. Survival of patients
  28.        classified according to the 1993 definition is much longer than for
  29.        those classified with the 1987 definition. Clinical status plays a major
  30.        role in predicting survival outcome among patients whose CD4+ levels
  31.        meet the new case definition.
